Wellness Pathway Health Services is committed to protecting the privacy of your Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and its implementing regulations.

We may use and disclose your PHI for treatment, payment, and healthcare operations purposes without your written authorization. Other uses and disclosures require your written authorization, which you may revoke at any time.

Your Rights Regarding Your PHI

You have the right to: request restrictions on uses and disclosures; receive confidential communications; inspect and copy your PHI; request amendments to your PHI; receive an accounting of disclosures; and receive a paper copy of this Notice.

We are required by law to maintain the privacy of your PHI, provide you with this Notice, and abide by the terms of this Notice currently in effect.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be retaliated against for filing a complaint.
